For your first visit, please arrive 30 minutes early to your first visit to fill out any necessary paperwork and change into appropriate clothing. Remember to bring your referral/ prescription for physical therapy, your insurance card or Worker's Comp info, a photo ID, and some comfortable and loose clothing that will allow you to easily expose the area being treated. We will assist you with understanding your insurance benefits for physical therapy.

A full hour or more is reserved for your first visit. You will meet with your therapist in a private room. During this time a complete history is taken, an evaluation is performed and treatment is started. We will strive to relieve your pain, as well as educate you about the nature of your problem and instruct you in activities and exercises to begin at home. At subsequent visits to Active Life Physical Therapy, your therapist may work with you to improve your function, range of motion, strength, and flexibility using weights and resistance equipment, as well as incorporating different balance and coordination training activities.

Being a preferred provider does not guarantee reimbursement from your insurance company. We recommend that you check with your individual plan for eligibility, requirements and policy limits for physical therapy services.

*For some insurance plans, we are out of network. We can bill these plans, but benefits will be processed as out of network. Please refer to your benefit package.
